New Patient Information
At Temas Eye Center, we would like to make your first visit as easy as possible. From here you can print out all of our "New Patient" forms. Completing the forms beforehand will expedite your visit. Please make sure that you bring the forms and your insurance cards with you to your appointment. Please arrive at least 15 minutes before your appointment so we can get you set up in our system.

Cancellation Policy
So that we may keep our schedule running smoothly, we ask that you give us at least a 24 hour advance notice of any need to cancel or reschedule an appointment.
"No Showing" for your appointment takes valuable appointment times away from other patients and reduces our efficiency. We do charge a $25.00 cancellation fee for any "No Show" after the first "No Show".
